Full Gospel Child Development Center Preschool

Limitations on Capacity and Ambulatory Status — Based on LPA's Calderon and Reyes observation, LPA's observed 11 preschool children and 3 toddlers. Facility is licensed for 10 preschool children and 5 toddlers. The facility representative did not comply with the section cited above in [1] out of [10] children present which poses an immediate health, safety or personal rights risk to persons in care.

Limitations on Capacity and Ambulatory Status — Based on observation and record review, the licensee did not comply with the section cited above in that facility license statesonly 10 preschool children and 5 toddlers in care and during today's visit LPA observed 15 preschool children in care which poses an immediate health, safety or personal rights risk to persons in care.

(C)Any unusual incident or child absence that threatens the physical or emotional health or safety of any child. The requirement was not met as evidance by: LPA Calderon observed workers that were repairing the back yard gate in the chlildren play area; LPA observed no child playing in area and LPA Calderon was informed of a incident that occured at the facility during the weekend, window doors of facility were broken by a unknown individual , police report number provided by Licensee A.Huffman and Per Huffman no children were present. LPA Calderon observed Glass area of the door to be broken and covered with trash bag. Per C.Seymour interview- in process of getting repaired.

Personnel Requirements — Based on LPA Calderon record review, there were no staff present that had a valid Pediatric First aid and cpr.

Personnel Requirements — Based on record review staff #1, staff #2 and staff #4 are missing LIC 702 Health Screening the licensee did not comply with the section cited above in [3] out of [4] children files reviewed which pose a potential health, safety or personal rights risk to persons in care.

Teacher Qualifications and Duties — Based on record review Staff #2 and Staff #3 do not have early education CDC units, the licensee did not comply with the section cited above in (2) out of [4] staff education records missing which poses/posed a potential health, safety or personal rights risk to persons in care.

Staff immunization requirements — Based on record review staff #1 did not have MMR, TB, TDAP immunization's on file, the licensee did not comply with the section cited above in [1] out of [4] persons immunization records poses a potential health, safety or personal rights risk to persons in care.

Carbon monoxide detectors required — Based on observation on 1/6/25, LPA Calderon and Tyler Reyes observed at 12:28pm carbon monoxide device without a battery not in device. The licensee did not comply with the section cited above in [1] out of [1] carbon monoxide device which poses a potential health, safety or personal rights risk to persons in care.

Disaster and Mass Casualty Plan — Based on record review and interview with staff Christian Saeymour unable to provide disaster and fire drill on visit date 1/6/25, the licensee did not comply with the section cited above in [1] out of [1] disaster and fire drill documentation which poses a potential health, safety or personal rights risk to persons in care.

Napping Equipment — Based on observation by LPA Calderon and Reyes on 1/6/25, the licensee did not comply with the section cited above in [15] out of [15] foldable mats for napping equipment, linen was observed touching each other, which poses/posed a potential health, safety or personal rights risk to persons in care.

Buildings and Grounds — Based on observation on 01/06/25, Outdoor play yard at 10:00am, LPA Calderon and Reyes observed on the right hand side of the shed pile of chairs either broken, rusted, and metal chair cap/ swivel glide was broken off . There was a metal pole sticking out measuring at approximately 8 inches. Outdoor fence on 2/20/25 had 3 hanging scaffolding and in addition LPA observed broken window near behind the cabinet located on the right hand side of the wall heater in the classroom. The licensee did not comply with the section cited above in [4] out of [4] broken chairs, metal pole, hanging scaffolding and broken window which poses a potential health, safety or personal rights risk to persons in care.

Fixtures, Furniture, Equipment and Supplies — Based on observation from visit 1/6/25 and 2/20/25 two trash bins were observed not having tight fitted lids. The indoor trash was observed having food waste in bin and per interview with Administrator Chrisitan Seymour outdoor trash used to place solid waste, the licensee did not comply with the section cited above in [2] out of [2] trash pins without tight fitted covers which poses a potential health, safety or personal rights risk to persons in care.

Food Service — Based on observation LPA did not see a posted food menu and interview with Administrator Chrisitan Seymour on 1/6/25 stated is unable to provide LPA with food menu has not created one yet, the licensee did not comply with the section cited above in [1] out of [1] food menu which poses/posed a potential health, safety or personal rights risk to persons in care.

Sign In and Sign Out — Based on record review and observation, the licensee did not comply with the section cited above in [2] out of [14] [children] present on 1/6/25 visit date, were not signed in on the roster, which poses a potential health, safety or personal rights risk to persons in care.

Drinking water lead testing — Based on observation and record review, the licensee did not comply with the section cited above in no record provided with evidence of lead test date and results which poses/posed a potential health, safety or personal rights risk to persons in care.

Disaster and Mass Casualty Plan — Based on observation and record review, the licensee did not comply with the section cited above in no record provided of conducted fire drills which poses/posed a potential health, safety or personal rights risk to persons in care.

Personnel Requirements — Based on record review, the licensee did not comply with the section cited above in at least one staff present with updated CPR/1st aid certificate which poses/posed a potential health, safety or personal rights risk to persons in care.

Personnel Records — Based on record review, the licensee did not comply with the section cited above in one staff recods being incomplete which poses/posed a potential health, safety or personal rights risk to persons in care.

Sign In and Sign Out — (a) In addition to the sign-in procedure requirement of Section 101226.1(b), the licensee shall develop, maintain and implement a written procedure to sign the child in/out of the child care center that shall, at a minimum, include the following: (1) The person who signs the child in/out shall use his/her full legal signature and shall record the time of day. Based on observation and record review, the licensee did not comply with the section cited above in two children missing on sign in and out sheet which poses/posed a potential health, safety or personal rights risk to persons in care.
