The requirement was not met as evidance by: LPA Calderon observed workers that were repairing the back yard gate in the chlildren play area; LPA observed no child playing in area and LPA Calderon was informed of a incident that occured at the facility during the weekend, window doors of facility were broken by a unknown individual , police report number provided by Licensee A.Huffman and Per Huffman no children were present. Buildings and Grounds — Based on observation on 01/06/25, Outdoor play yard at 10:00am, LPA Calderon and Reyes observed on the right hand side of the shed pile of chairs either broken, rusted, and metal chair cap/ swivel glide was broken off . There was a metal pole sticking out measuring at approximately 8 inches. Outdoor fence on 2/20/25 had 3 hanging scaffolding and in addition LPA observed broken window near behind the cabinet located on the right hand side of the wall heater in the classroom. The licensee did not comply with the section cited above in [4] out of [4] broken chairs, metal pole, hanging scaffolding and broken window which poses a potential health, safety or personal rights risk to persons in care.
