Responsibility for Providing Care and Supervision — Based on record review and interviews, on 7/7/26 at approximately 11:40am, while playing in the upper yard, Child 1 (C1) injured their mouth with a wooden block and the two staff present did not visually observe the injury therefore C1 was not supervised during the injury which poses an immediate health and safety right risk to children in care.

Staff immunization requirements — Based on record review, the licensee did not comply with the section cited above in Staff 1 (S1) does not have proof of immunization against measles and pertussis which poses/posed a potential health, safety or personal rights risk to persons in care.
Health-Related Services — Based on observation and record review, the licensee did not comply with the section cited above in LPA observed (1) expired medication which poses/posed a potential health, safety or personal rights risk to persons in care.
Drinking water lead testing — Based on observation, interview and record review, the licensee did not comply with the section cited above. The facility could not provide a copy of the completed lead testing and LPA did not observe lead testing completed on the waterboard website. This poses a potential health, safety or personal rights risk to persons in care.
