Licensing Program Analyst (LPA) observed that the the three staff on site did not have their immunization records on file as well as a Health Screening Report. In addition, staff #1 was missing her educational record. Staff #3 was missing Mandated Reporter Training.
Mandated reporter training — Based on record review, the licensee did not comply with the section cited above which poses a potential health, safety or personal rights risk to persons in care. During employee record review, Licensing Program Analyst (LPA) observed that two substitute staff members were missing Mandated Reporter Training for Child Care Providers.
