No child(ren) shall be left without the supervision of a teacher at any time, except as specified in Sections 101216.2(e)(1) and 101230(c)(1). Supervision shall include visual observation. Based upon staff interviews, Staff entered the classroom without a child and realized there was a child missing the after door was closed. Teacher-Child Ratio — Based on interview statements, it was revealed that the facility was out of ratio on April 10, 2025, which poses a potential risk to the health, safety, and personal rights of children in care.
Responsibility for Providing Care and Supervision — Based on staff statements, it was revealed that staff failed to visually monitor all children during outside activities, which poses a potential risk to the health, safety, and personal rights of children in care.
To be free from corporal or unusual punishment, infliction of pain, humiliation, intimidation, ridicule, coercion, threat, mental abuse, or other actions of punitive nature… Based upon staff interviews, staff aggressively grabbed the child’s face and the incident was not address immediately. Due to lack of personal rights this poses as a potential risk to the health and safety of the children in care.
Personal Rights — Based on interviews and record review, it was revealed that an incident occurred on 07/28/2023, where Staff #1 and Staff #2 were engaged in a verbal altercation in front of day care children during operating hours. This poses a potential health, safety or personal rights risk to persons in care.
