Buildings and Grounds — Based on observation, the licensee did not comply with the section cited above which poses a potential health, safety or personal rights risk to persons in care. During the indoor inspection, Licensing Program Analyst (LPA) observed that the main blue carpet in Clasrooms 1 and 2 had dark marks and staining throughout. LPA observed some dead insects/flies along the bottom of the windows/window sills in Room 2. LPA also observed some ants on the cubbies near the front entrance in Room 1 and around the outside of the door in Room 2. Lastly, LPA observed a strong odor of urine coming from the bathroom in Room 1.
Fixtures, Furniture, Equipment and Supplies — Based on observation, the licensee did not comply with the section cited above in LPA observed that Classroom 1 and Classroom 2 did not have a trash bin with a tight fitting cover for solid wastes. LPA observed trash cans in classroom 1 and classroom 2 with food waste, which poses/posed a potential health, safety or personal rights risk to persons in care.
Criminal background check — Based on observed, the licensee did not comply with the section cited above in LPA observed that an uncleared adult was in the Staff office area in Classroom 1. LPA was informed by Staff #3 that adult is not fingerprint cleared or a volunteer. Uncleared adult left classroom/ school grounds, which poses/posed a potential health, safety or personal rights risk to persons in care.
Personnel Records — Based on record review, the licensee did not comply with the section cited above in that LPA observed that Staff #1 and Staff #2 did not have a recent Mandated Reporter. LPA also observed that Staff #5 did not have their Physicians Report, which poses/posed a potential health, safety or personal rights risk to persons in care.
Personal Rights — Based on record review, the licensee did not comply with the section cited above in that a total of 4 children did not have the Parent and Personal Rights on file. Children consist of Child #2, Child #3, Child #4, and Child #5, which poses/posed a potential health, safety or personal rights risk to persons in care.
Disaster and Mass Casualty Plan — Based on record review, the licensee did not comply with the section cited above in LPA was informed by Staff #1 and Staff #3 that center had not conducted a disaster drill since the start of school, which poses/posed a potential health, safety or personal rights risk to persons in care.
Reporting Requirements — Based on interview, the licensee did not comply with the section cited above in that the classroom AC units were not working properly and that the School had provided Classroom 1 and Classroom 2 with space heaters to warm up the classrooms. LPA was informed that AC unit will be replaced, which poses/posed a potential health, safety or personal rights risk to persons in care.
