Infant Safe Sleep — Based on observation and record review, the licensee did not comply with the section cited above. LPA observed two infants sleeping on their tummies and do not have a Safe Sleep plan on file which poses an immediate health, safety or personal rights risk to persons in care.
Staffing Ratio and Capacity — Based on observation and record review, the licensee did not comply with the section cited above. The facility was caring for four preschool children and five infants. Licensee was out of ratio which poses an immediate health, safety or personal rights risk to persons in care.

Family home — safe toys and play equipment — Based on observation, the licensee did not comply with the section cited above. LPA observed a trampoline that had frayed material and broken plastic on the bottom that is a potential hazard for children. A slide from a nearby play structure was propped up to slide into the trampoline opening and not part of the manufacture's recommendations which poses/posed a potential health, safety or personal rights risk to persons in care.
Alterations to Existing Building or Grounds — Based on observation and interview, the licensee did not comply with the section cited above LPA observed licensee changed the infant napping room to a previously reported off limits bedroom which poses/posed a potential health, safety or personal rights risk to persons in care.

Licensee stated she was not aware she needed to practice drills which poses/posed a potential health, safety or personal rights risk to persons in care.
Infant Safe Sleep — Based on observation, the licensee did not comply with the section cited above. One infant had a blanket in the play yard in lieu of a fitted sheet which poses/posed a potential health, safety or personal rights risk to persons in care.

LPA observed licensee assistant Maria does not have immunization records on file which poses/posed a potential health, safety or personal rights risk to persons in care.
