Responsibility for Providing Care and Supervision — licensee failed to ensure that children in care were adequately supervised as child #1 exposed his genitalia on multiple occasions to staff and other children on or around June and July 2024. This poses an immediate health and safety risk in that the licensee shall ensure that children are afforded adequate supervision while in care

Responsibility for Providing Care and Supervision — staff was in between the door of the classroom and playground at times, they were not readily available to assist, care and meet the children’s needs.
Buildings and Grounds — LPAs observation and staff interviews noted on LIC9099 and LIC9099-C, poses a potential risk to the health, safety, or personal rights of children in care.
Criminal Record Clearance — Based on a record review, the licensee did not comply with the section cited above. Staff #1, Staff #2 and Staff #3, who are present today, are fingerprint cleared, but are not associated to the facility, which poses a potential health, safety or personal rights risk to persons in care.
