Personal Rights — Based on information gathered on 4/30/2025 video footage show staff (S-1) was redirecting child (C-1) during in outdoor incident were staff (S-1) mishandled child (C-1) resulting in bruising and scratches. This poses an immediate risk to the health, safety, and personal rights of children in care.

Responsibility for Providing Care and Supervision — Based on interview, the licensee did not comply with the section cited above by not properly supervising children during transition time from the playground to the classroom, which poses a potential health, safety, or personal rights risk to persons in care.

Drinking water lead testing — Based on interview, the licensee did not comply with the section cited above which poses a potential health, safety or personal rights risk to persons in care. Facility has not completed water lead testing by the deadline.

Mandated reporter training — Based on observation, interview and record review, the licensee did not comply with the section cited above for staff S3 which poses a potential health, safety or personal rights risk to persons in care.

Staff immunization requirements — Based on observation, interview and record review, the licensee did not comply with the section cited above for staff S2 and S3 which poses a potential health, safety or personal rights risk to persons in care.

Health-Related Services — Based on observation and interview, the licensee did not comply with the section cited above for child C1 which poses a potential health, safety or personal rights risk to persons in care. LPA observed an Epi Pen for child C1 which has expired and has not been returned or renewed by facility.

Reporting Requirements — Based on interview, the licensee did not comply with the section cited above which poses a potential health, safety or personal rights risk to persons in care. Director had not submitted a new director packet to Child Care Licensing within 10 days of change of director.
