(6) The toddler program shall be conducted in areas separate from those used by older or younger children... — Based on observation, interviews, and record review, the licensee did not comply with the section cited above by commingling C1, C2, and C3 between preschool and toddlers class which poses a potential health, safety or personal rights risk to persons in care.

Modifications to Infant Needs and Services Plan — Based on observation record review, the licensee did not comply with the section cited above by not having the infants needs and services signed by the centers representative which poses/posed a potential health, safety or personal rights risk to persons in care.
Outdoor Activity Space — Based on observation, the licensee did not comply with the section cited above by not ensuring that the outside playground area is maintained, clean, and in good condition for children to play freely which poses/posed a potential health, safety or personal rights risk to persons in care.
Food Service — Based on interview and record review, facility did not offer the morning snack that was listed on the menu, which poses a potential health and safety risk to children.
Parents shall give permission for the placement of their children in the toddler program. — Based on record review, 6 out of 8 children did not have written permission from parent to place child in the toddler program, which poses a potential health and safety risk to children in care.
The maximum group size, with two teachers, or one fully qualified teacher and one aide, shall not exceed 12 toddlers. — Based on observation, LPA observed on 10/19/2023 that there were three staff with 13 toddlers, which poses a potential health and safety risk to children in care.
