At the time of observation, the doors were not locked, making the detergent and bleach accessible to children in care. Immunization records (pertussis & measles) were missing for Staff 1, which poses a potential health, safety or personal rights risk to persons in care.
Infant Safe Sleep — Based on observation and record review, the licensee did not comply with the section cited above. An Individual Infant Sleeping Plan (LIC 9227) was missing for Child 1 which poses a potential health, safety or personal rights risk to persons in care.
