LPA did not observe the alarm working at time of LPA visit.
Criminal background check — Based on interview and record review, the licensee did not comply with the section cited above as LPA observed one assistant without criminal record clearance. Licensee stated assistant turned 18 recently. LPA record reviews showed assistant turned 18 in August, and has been employed at the facility since July 2024, which poses an immediate health, safety or personal rights risk to persons in care.
