Responsibility for Providing Care and Supervision — Based on interview and record review, a child fell out of a wagon, injuring their left leg and no staff visually observed the incident as it took place. This poses/posed an immediate health, safety, or personal rights risk to persons in care.
Health-Related Services — Based on interview and record review, a child fell and injured their leg on the playground. The child had difficulty standing and showed signs of limping and of being injured. Although the child showed signs of injury no medical attention was sought out for child. This poses/posed an immediate health, safety, or personal rights risk to persons in care.
Responsibility for Providing Care and Supervision — Based on interview, record review, and self report from facility, the licensee did not comply with the section cited above in that an inappropriate action occurred between two children, while there was no staff observing them. No injuries were reported. This poses/posed an immediate health, safety, or personal rights risk to person in care.
Buildings and Grounds — Based on observation, the licensee did not comply with the section cited above, disenfect, scissors and staff personal items which poses an immediate health, safety risk to persons in care.

Mandated reporter training — Based on record review and interview, the licensee did not comply with the section cited above in that 1/5 staff members (Staff 3 [S3]) did not renew their mandated reporter training every two years, their last mandated reporter training expired 2/28/2026. This poses/posed a potential health, safety or personal rights risk to persons in care.
Personnel Requirements — Based on observation and interview, the licensee did not comply with the section cited above in that there was 1 volunteer with current criminal record clearance or exemption that was not supervised while providing care and supervision to children in care. This volunteer additionally had enough child care related units to be considered a fully qualified teacher if they were employed by this facility. This poses/posed a potential health, safety or personal rights risk to persons in care.
Personal Rights — Based on interview & record review, the facility did not prevent a child in care from being "smacked" on the butt with a toy, multiple times, by a temporary staff member. The "smacks" would not be considered "gentle" and would not be considered "hard" either. This poses/posed a potential health, safety, or personal rights risk to persons in care.
Buildings and Grounds — Based on observation the licensee did not comply with the section cited above which poses a potential health, safety or personal rights risk to persons in care. LPA observed during the tour in one of the classrooms a mop with cleaning solution attached to it was on the floor in the bathroom and accessbile to the children. LPA observed no children were in the bathroom at the time of inspection.
