Responsibility for Providing Care and Supervision for Infants — Based on observation and interview, the licensee did not comply with the section cited above. At 11:00am, LPA walked into the infant with director and observe an infant alone on the swing. Photo was taken. While staff was in the nap room with 8 infants/toddlers which poses an immediate health, safety or personal rights risk to persons in care.
Limitations on Capacity and Ambulatory Status — Based on observation and interview, the licensee did not comply with the section cited above. Director stated that infants and toddlers are always together and they share the rooms which poses an immediate health, safety or personal rights risk to persons in care.

Sign In and Sign Out — Based on observation and record review, the licensee did not comply with the section cited above in six children missing on sign in and out sheet which poses/posed a potential health, safety or personal rights risk to persons in care.
