Reporting Requirements — Based on Licensee admittance and interview, the licensee did not comply with the section cited above. The Licensee stated she recieved notification that a child who was injured in care received medical treatment and it was not reported to the Department. This is a potential risk to the health, safety or personal rights risk to persons in care.
Mandated reporter training — Based on observation and Licensee's statement that her Mandated Reporter Training certificate expired on 02/04/22 the licensee did not comply with the section cited above in that her Mandated Reporter Training certificate is expired, which poses as a potential health, safety or personal rights risk to persons in care.
