Personal Rights — LPA's observation, interviews conducted and record review. Facility did not comply with Parents request - no diary to child #1. According to facility staff was not aware goldfish contained diary. LPA observed a sign in the classroom listed with child#1's food restriction which included goldfish. This poses a potential health and safety risk to children in care.
