Mandated reporter training — Based on record review, the licensee did not comply with the section cited above in 3 staff not having proof of Child Abuse Mandated Certificate which poses a potential health, safety or personal rights risk to persons in care.
Staff immunization requirements — Based on record review the licensee did not comply with the section cited above in having 7 staff members not submit prood of immunizations which poses a potential health, safety or personal rights risk to persons in care.
