Teacher Aide Qualifications and Duties — LPA observation and interviews. During inspection LPA witnessed Staff #1 being left alone with preschool children even though Staff #1 doesn’t meet the required qualifications of a preschool teacher. Drinking water lead testing — Based on record review, the licensee did not comply with the section cited above. Licensee did not conduct lead testing by deadline which poses a potential health, safety or personal rights risk to persons in care.
Buildings and Grounds — Based on observation, the licensee did not comply with the section cited above. LPA observed portable fans, blinds, restroom stalls walls need to be dusted and cleaned. Carpet near the restroom is begining to rip and needs repair or a area rug which poses a potential health, safety or personal rights risk to persons in care.
Mandated reporter training — Based on record review, the licensee did not comply with the section cited above. LPA observed 2 staff files failed to have an updated mandated reporter training which poses a potential health, safety or personal rights risk to persons in care.
Food Service — Based on observation, the licensee did not comply with the section cited above. LPA observed no menus or daily activity schedules posted which poses a potential health, safety or personal rights risk to persons in care.
Child's Records — Based on record review. Licensee was unable to provide copies of Children's current Roster. Which poses a potential health, safety or personal rights risk to persons in care.
Operating without a license — Based on records that licensee had not paid annual licensing fees. Which poses a potential health, safety or personal rights risk to persons in care.
Buildings and Grounds — Based on observations. LPA observed that various areas of the facility in the preschool side need maintence as stated in the LIC809 Report. Which poses a potential health, safety or personal rights risk to persons in care.
