Family home — admission procedures and parent rights — Based on record review, the licensee did not comply with the section cited above. 1 out of 4 children did not have the LIC995A signed by a parent/authorized representative. Although the form was on file, it was not filled out and signed off by the parent/authorized representative. This poses/posed a potential health, safety or personal rights risk to persons in care.
Infant Safe Sleep — Based on record review, the licensee did not comply with the section cited above. There were no 15 minute checks available for review. Assistant stated that they do them, but could not locate where they are stored. This poses/posed a potential health, safety or personal rights risk to persons in care.
Where children are less than five years old are in care, stairs shall be fenced or barricaded. This was not met as evidenced by: LPA's immediately observed the gate to the stairs wide open, making the stairs accessible to the children in care. This poses a potential risk to the health and safety to children in care.
