Based on interview, LPA learned that one teacher was supervising 18 children while another staff took a bathroom break which places the classroom out of ratio.This poses an immediate risk to the health, safety and personal rights of children in care.

Food Service — Volunteer provided C1 with a food item containing contents that were documented to cause C1 a non-life threatening allergic reaction. This poses a potential health and safety risk to children in care.
Health & Safety: A licensed child day care facility shall provide to the parents or guardians of each child receiving services in the facility copies of any licensing report that documents any Type A citation that represents an immediate risk to the health, safety, or personal rights of children in care as set forth in paragraph (1) of subdivision (a) of Section 1596.893b.(2) Upon enrollment of a new child in a facility...(3)recipient to sign(4)keep verification of receipt on child's file... Based on record review, the licensee did not comply with the section cited above in 4 out of 5 files which poses/posed a potential health, safety or personal rights risk to persons in care.
Reporting Requirements — LPA observed that incident was reported to parent but reporting requirements to the Childcare Licensing Department were not followed.
Responsibility for Providing Care and Supervision — Based on observation, LPAs observed that a child was behind the slide of the play structure. A staff did went over to the child. There were two staff present, but cannot visually see the child from where they were positioned. This poses a potential health and safety risk to children.
