Teacher Qualifications and Duties — Based on record review, the licensee did not comply with the section cited above inone of five staff files reviewed. Where proof of enrollment in ECE curriculum course was not provided for S1 during inspection, Which poses a potential health, safety or personal rights risk to persons in care.
101239 Fixtures, Furniture, Equipment and Supplies (e) (4) All toilets, handwashing and bathing facilities shall be maintained in safe and sanitary operating condition. This requiremnt was not met as evidence by....... Based on observation and interviews classroom # 6 had 19 children present on the date of the visit with only one working toilet. Interviews disclosed most of the time children are to dispose of used toilet paper in the trashcan for both urination and bowel movements. Which poses a potential health risk to children in care.
Child's Records — Based on interview and record review, the licensee did not comply with the section cited above in one out of ten child files reviewed. Unavialable was a signed LIC 627 for Child 1. Which poses a potential health, safety or personal rights risk to persons in care.
Activities/Napping — Based on observation and interviews, it was revealed at times during nap time, only one staff member will be available providing supervision for more than twelve children during nap, while not all children are asleep or on their cots. On two occasions, LPA observed three of four classrooms out of ratio.
