Staffing Ratio and Capacity — Based on observations, record reviews, and interviews, the licensee provided care for five infants and 10 preschool-aged children on 03/19/2025, and three infants (up to 24 months) and 10 preschool-aged children (ages 2 to 5) on 04/03/2025. The licensee was out of compliance with both ratio and capacity requirements on both dates. This poses an immediate risk to the health, safety, or personal rights of children in care.

Family home — home free of safety hazards — Based on LPA observations and licensee interview, the licensee did not comply with the section cited above. LPA observed in accessible living room a pet bird and its cage against the living room wall. LPA observed underneath the cage, white bird feces and food on the floor which poses/posed a potential health, safety or personal rights risk to persons in care.
(a) The licensee shall be present in the home and shall ensure that children in care are supervised at all times. When circumstances require the licensee to be temporarily absent from the home, the licensee shall arrange for a substitute adult to care for and supervise the children during his/her absence. Temporary absences shall not exceed 20 percent of the hours that the facility is providing care per day. Based on LPA interviews and information received, Licensee did not provide adequate supervision for children in care. Licensee did not comply with the section cited above which poses a potential health, safety, or personal rights risk to persons in care.
