Drinking water lead testing — Based on observation and interview, the licensee did not comply with the section cited above in faciilty was not able to provide proof of the facility's water getting tested for lead which poses a potential health, safety or personal rights risk to persons in care.
Personal Rights — a facility self report incident, interviews and LPA observing a video footage in a classroom that staff # 1 pulled out a Motnessori material out of a 23 months old child's hands inappropriately in an unkind way. This is potential risk to the health and safety and personal rights of children in care.
Mandated reporter training — Based on observation, interview, record review, the licensee did not comply with the section cited above. During staff file reviews, LPA observed mandated reporter certifications for 3 out of 5 staff were expired which poses/posed a potential health, safety or personal rights risk to persons in care.
Notice to parents that site visit reports are available — Based on observation, interview, and record review, the licensee did not comply with the section cited above. LPA discovered 11 infant children and 13 toddler age children were missing the LIC 9224 Acknowledgement of Receipt of Licensing Report dated 4/12/2023 which poses/posed a potential health, safety or personal rights risk to persons in care.
