Infant Safe Sleep — Based on observation, the licensee did not comply with the section cited above in having loose articles and objects like blankets, pillows, and a bottle in the pack and plays that were utilized by infants which posed a potential health, safety or personal rights risk to persons in care.
Mandated reporter training — Based on observation and file review, the licensee did not comply with the section cited above in not ensuring S1 renewed their mandated reporter training and S2 did not have their mandated reporter training which poses a potential health, safety, or personal rights risk to persons in care.
Infant Safe Sleep — Based on observation, the licensee did not comply with the section cited above which posed a potential health, safety or personal rights risk to persons in care. There were two infants in care that were sleeping in a pack and play with pillows underneath their heads and blankets on each side of their body.
Infant Safe Sleep — Based on observation, the licensee did not comply with the section cited above which posed a potential health, safety or personal rights risk to persons in care. The licensee has not condcuted a sleep log for sleeping infants in care.
Family home — home free of safety hazards — Based on LPAs observation, the licensee did not comply with the section cited above where a kitchen cabinet's lock was broken where cleaning agents were observed to be present, and an outside shed doors were left without a lock where dangerous chemicals were stored which poses/posed a potential health, safety or personal rights risk to persons in care.
Immunizations — Based on record review the licensee did not comply with the section cited above where the licensee did not maintain C1, and C2's immunizations. This poses/posed a potential health, safety or personal rights risk to persons in care.
Personal Rights — Based on interviews conducted, staff were found to have grabbed children by the arm to lead them to a time-out. This is a potential personal rights risk to children in care.
Personal Rights: (a) Each child receiving services from a family child care home shall have certain rights that shall not be waived or abridged by the licensee regardless of consent or authorization from the child's authorized representative. These rights include, but are not limited to, the following: (1) To be treated with dignity in his/her personal relationship with staff and other persons. The requirement was not met as evidenced by: Based on interviews conducted, it was found that staff have yelled at the children. This potentially is a personal rights violation for children in care.
Personal Rights — Based on photographic evidence obtained, LPA found that there was a step missing to a ladder, and several bikes in disrepair. This is a potential health and safety risk to children in care.
Family home — children supervised at all times — Based on staff interview, the Licensee was absent for more than 20% of the hours of the day, and therefore the regulation was not met. This is a potential health and safety and/or personal rights risk to children in care.
