NO fire drill(s) have been conducted since date of licensure, 11/18/2025 which poses/posed a potential health, safety or personal rights risk to persons in care.

C1, C2 and C3 does NOT have LIC9227, LIC700, LIC627, LIC282, LIC995A and immunizations in file which poses/posed a potential health, safety or personal rights risk to persons in care.
Infant Safe Sleep — Based on LPA records review and Licensee interview, the licensee did not comply with the section cited above. C1 and C2 does NOT have 15-minute sleep logs. In addition, C4 has NOT had 15-minute sleep logs since 01/27/2026. LPA stated and Licensee confirmed 02/2026, 03/2026 and 04/2026, C4 does NOT have logs which poses/posed a potential health, safety or personal rights risk to persons in care.
