Personal Rights — Based upon observation, interview, and records the facility did not provide a safe environment for children in care as bitting incidents continued to occur which posed an immediate health, safety, and personal rights risk to children in care.

Personal Rights — Based on interviews and video footage review, C1 was handled in a rough manner by S1 who first yelled at C1 three times, a toy was pulled away from C1 by S1 and C1 was grabbed and sat on a chair by S1. This poses an immediate health and safety risk for children in care.

Reporting Requirements — Based upon observation, interview, and records the facility did not report all biting incidents to C1's parent which posed a potential health, safety, and personal rights risk to persons in care.

Child care center... specified in (d)(1) below, a report shall be made to the Department by telephone or fax within the Department's next working day and during its normal business hours...Any unusual incident or child absence that threatens the physical or emotional health or safety of any child. Based upon observation, interview, and records the facility did not report excessive biting incidents to department which posed a potential health, safety, and personal rights risk to persons in care.

Posting of License — Based on observation, interview, record review, the licensee did not comply with the section cited above in the facility did not have license or required postings posted in a prominent, publicly accessible location in the center, which poses/posed a potential health, safety or personal rights risk to persons in care.

Disaster and Mass Casualty Plan — Based on observation, interview, record review, the licensee did not comply with the section cited above in as the facility did not document Emergency Disaster Drill, which poses/posed a potential health, safety or personal rights risk to persons in care.

Mandated reporter training — Based on observation, interview, record review, the licensee did not comply with the section cited above in 1 out of 5 members did not obtain proof of current childcare mandated reporter training certificate, which poses/posed a potential health, safety or personal rights risk to persons in care.

Child's Medical Assessments — Based on observation, interview, record review, the licensee did not comply with the section cited above in 1 out of 9 children did not have a completed physician’s report for LPA to review which poses/posed a potential health, safety or personal rights risk to persons in care.

Personal Rights — Based on observation, interview, record review, the licensee did not comply with the section cited above in 1 out of 9 children obtain expired medication at the facility, which poses/posed a potential health, safety or personal rights risk to persons in care.

Mandated reporter training — Based on observation and record review, the licensee did not comply with the section cited above in 2 out of 7 staff are missing current mandated reporter traiing. Which poses/posed a potential health, safety or personal rights risk to persons in care.

Admission Policies — Based on interviews and record review, Parent #1 stated she did not receive the substantiated complaint and LPA did not observe a signed LIC9224 in child's file. This poses a potential health and safety risk to children in care.

Personal Rights — Based on interviews conducted S4 to S7 have at one time yelled at one another in the presence of children. Per S4-S7 the yelling has been observed/done by the above staff. This poses a potential health and safety risk for children in care.

Reporting Requirements — Based record review and director disclosure, the incident was not reported to the department. This poses a potential and health and safety risk to children in care.
