Criminal background check — Based on observation, interview, record review, the licensee did not comply with the section cited above S#1, S#2 and S#3 did not have criminal record clearance, which poses an immediate health, safety or personal rights risk to persons in care.

Storage Space — Based on observation and interviews the licensee did not comply with the section cited above LPA observed cleaning solutions, poisions and medication made accessiable to the children in care, which poses an immediate health, safety or personal rights risk to persons in care.

Buildings and Grounds — Based on observation, interview, record review, the licensee did not comply with the section cited above in, LPA observed the pond to be enclosed by a makeshift orange safety netting gate which is not secure and is accessible to the children as well as the classroom door have immidiate access to the pond, which poses an immediate health, safety or personal rights risk to persons in care.

Family home — in-ground pool drowning prevention requirements — Based on observation interview record review, the licensee did not comply with the section cited above in not performing daily inspections which poses a potential health, safety or personal rights risk to persons in care.

Fixtures, Furniture, Equipment and Supplies — Based on observation and interviews the licensee did not comply with the section cited above, LPA observed a ripped vinyl diapering mat in the changing table in the classroom and have visible soil marks, additionally, a pile of transparency paper sheets were stacked one under another on the mat, which poses a potential health, safety or personal rights risk to persons in care.

Napping Equipment — Based on observations, the licensee did not comply with the section cited above in, LPA observed napping equipment to be in the form of cots to have sheets and blankets attached to them and were observed to be touching, which poses a potential health, safety or personal rights risk to persons in care.

Buildings and Grounds — Based on observation and interview, the licensee did not comply with the section cited above in LPA observed one out of service toilet in Ms. Pats classroom as well as one sink out of use and no water coming out which poses a potential health, safety or personal rights risk to persons in care.

Fixtures, Furniture, Equipment and Supplies — Based on observations, the licensee did not comply with the section cited above in, LPA Perry did not observe step trash bin with a lid or trash bins in the classrooms with tight fitting lids to dispose of food, which poses a potential health, safety or personal rights risk to persons in care.

Fixtures, Furniture, Equipment and Supplies — Based on observations and iinterviews, the licensee did not comply with the section cited above in, LPA Perry observed a portable AC/Heater accessible to the children. LPA touched the unit and it was observed to be hot to the touch which poses a potential health, safety or personal rights risk to persons in care.

Activities/Napping — Based on observations, the licensee did not comply with the section cited above in, during naptime LPA observed teacher to be out of ratio which poses/posed a potential health, safety or personal rights risk to persons in care.

Mandated reporter training — Based on observations, interviews and record reviews, the licensee did not comply with the section cited above in, LPA observed 6 out of 6 staff files not to have a current Mandated Reporter Training on file, which poses a potential health, safety or personal rights risk to persons in care.

Child Care Center Director Qualifications and Duties — Based on observations, interviews and record review, the licensee did not comply with the section cited above in, LPA Perry did not observe Preventative Health and Safety certification on file for director not did LPA observe Directors Community Care Licensing Orientation Certification, which poses a potential health, safety or personal rights risk to persons in care.

Personnel Requirements — Based on observations, interviews and record reviews, the licensee did not comply with the section cited above in, LPA observed 6 out of 6 staff files did not have Pediatric First Aid/CPR certification current which poses a potential risk to the health and safety of the children in care, which poses a potential health, safety or personal rights risk to persons in care.
