Per licensee, they use the bucket to collect the water from the sink until the pluber comes to fix the sink. This poses a potential health, safety or personal rights risk to persons in care.
Mandated reporter training — Based on record review and interview, the licensee did not comply with the section cited above in 2 out of 3 staff did not renew their mandated reporter training. During record review LPAs did not observe a valid certificate in licnesee and staff 1's file. Per licensee, they did not do the training. which poses a potential health, safety or personal rights risk to persons in care.
