Personal Rights — Based on observation, interview, and record review, adults providing care and supervision fail to prevent daycare children from harming other children in care. Confidential interviews revealed C2 was flipped out of a highchair on 02/05/2025, by C1. Staff confirmed children fight at the daycare often, C4 and C5 was injured and had to seek medical attention. C3 informed that when C1 misbehaves “we have to beat him up” which poses a potential health, safety or personal rights risk to persons in care.
Reporting Requirements — The licensee did not comply with the section cited above by failing to timely report incident the that occurred on 02/05/2025 when (C1) pushed Child 2 (C2) out of the highchair and (C2) fell and hit head on the floor, or following the occurrence during the operation of the facility, which poses a potential health, safety or personal rights risk to persons in care.
