Infant Care General Sanitation — Based on observation, the licensee did not comply with the section cited above in the LPA observed the changing table equipment and mat to be unclean with visable debris and the area carpet rugs to be unclean, from spills of unknown substance and dirt build up, which poses a potential health, safety or personal rights risk to persons in care.
Reporting Requirements — iBased upon LPA record review and interviews conducted the facility did not report to the department an incident for child injury occurring on 09/04/24 that resulted in follow up medical care. This is a potential health and safety risk to children in care.
