Personal Rights — On October 22, 2025, The Licensee was found to be under the influence of alcohol, testing at a blood alcohol level of approximately .19, while supervising children in care. Child 1 (C1), an infant, was found to be unresponsive while in care. The infant has since passed away.

Reporting Requirements — A medical emergency occurred on October 22, 2025 while Licensee was under the influence of alcohol. Licensee failed to report an unusual incident to the Department that threatened the physical health and/or safety of a child in care.
