Buildings and Grounds — Based on observation and interview, the licensee did not comply with the section cited above in that Clorox cleaner, bleach, all purpose cleaner were observed in the infant room inside of the bathroom underneath the sink in an unlock cabinet, two rolls of trash bags in a drawer, two containers of Clorox disinfecting wipes in a corner next to the toilet, a Swiffer mop with liquid cleaner behind the bathroom door which poses a potential health, safety or personal rights risk to persons in care.
Outdoor Activity Space for Infants — Based on observation, interview and record review, the licensee did not comply with the section cited above in that LPA observed 11 preschool children on the playground exclusive for infant's use which poses a potential health, safety or personal rights risk to persons in care.
Mandated reporter training — Based on interview and record review, the licensee did not comply with the section cited above in that Mandated Reporter Training Certification for S1 expired on 8/7/2024 which poses a potential health, safety or personal rights risk to persons in care.
Modifications to Infant Needs and Services Plan — Based on interview and record review, the licensee did not comply with the section cited above in the Infant Needs and Service Plans for C4, C5, C6, C7 and C8 were not updated quarterly which poses a potential health, safety or personal rights risk to persons in care.
Infant Care Food Service — Based on observation and interview, the licensee did not comply with the section cited above in that a bottle of breast milk located in the refrigerator for C1 was not labeled with the name or date which poses a potential health, safety or personal rights risk to persons in care.
Alterations to Existing Buildings or New Facilities — Based on observation, interview and record review, the licensee did not comply with the section cited above in that a portion of the infant playground has been added without notification to the Department which poses a potential health, safety or personal rights risk to persons in care.
Infant Care Center Fixtures, Furniture, Equipment, and Supplies — Based on analyst observation, the licensee did not comply with the section cited above as two prohibited baby rockers were found in the non-mobile infant room which poses/posed a potential health, safety or personal rights risk to children in care.
Carbon monoxide detectors required — Based on analyst observation, the licensee did not comply with the section cited above as the infant classroom was not found to currently have an installed carbon monoxide detector which poses/posed a potential health, safety or personal rights risk to children in care.
Infant Needs and Services Plan — Based on analyst record review, the licensee did not comply with the section cited above as they have not been maintaining the LIC 9227 Individual Infant Sleeping Plan for any of their enrolled infants which poses/posed a potential health, safety or personal rights risk to children in care.
