Facility did not do lead testing before January 1, 2023 which poses a potential health, safety or personal rights risk to persons in care.

This requirement was not met by: Based on interview and LPA observation, the ceiling in blue room is leaking and peeling. There is some peeling on the ceiling on yellow room and the green room was leaking last week and the light covers have water. This poses a potential risk to the health and safety of children in care.
