Mandated reporter training — During record review, Licensee stated that she was unsure if her mandated reporter training was expired and was unable to provide documentation showing the last date it was completed. The licensee did not comply with the section cited above which poses a potential health, safety or personal rights risk to persons in care.
Infant Safe Sleep — During review of records, Licensee did not have documentation of the 15 minute sleep logs for all infants in care. Licensee stated she has been checking on the infants while they sleep, however she has not been documenting the checks, the licensee did not comply with the section cited above which poses a potential health, safety or personal rights risk to persons in care.
