During the staff file review, the LPA was unable to review a current Mandated Reporter certificate for S2 and S3. S2 stated the trainings were completed, however, the certificates could not be pulled up on the computer.
Infant Safe Sleep — Based on the record review, the Licensee did not meet the above regulation which poses a potential health & safety risk to the children in care. During the children’s file review, sleep logs for C2.
Immunizations — Based on the record review, the Licensee did not meet the above regulation which poses a potential health risk to the children in care. During the children’s file review the LPA was unable to review immunization for C1.
