Licensee stated that the female adult assisted her years ago but as of now is not associated and is not approved by the department. This poses an immediate health, safety or personal rights risk to persons in care.

During inspection LPA observed gas can, laundry detergent, and medication that is accessible to children in care. This which poses a potential health, safety or personal rights risk to persons in care.
Mandated reporter training — Based on interview and record review, the licensee did not comply with the section cited above. Licensee stated that she has completed training but unfortuantely she could not provide certificate on todays date. This poses a potential health, safety or personal rights risk to persons in care.
Staff immunization — influenza, pertussis and measles — Based on record review, the licensee did not comply with the section cited above. Licensee was unable to provide immunization record for assistant. This poses a potential health, safety or personal rights risk to persons in care.
