Personal Rights — Based on interviews and observations children are being handled in a rough manner, and not provided a safe environment. This poses a potential health, safety or personal rights risk to persons in care.

Personal Rights — Based on interviews and observations Licensee has been observed pinching a child in care as well as causing injury to child(ren) in care. This poses a potential health, safety or personal rights risk to persons in care.

Criminal Record Clearance — Based on record review and interviews conducted A1 was working in the facility without a clearance from 05/20/24 -06/05/2024. This poses a immediate health and safety risk to children in care.

Personal Rights — Based on interview and observation children have been allowed to attend the facility while ill. Interviews with staff, children and adults all revealed that the Licensee and S4 speak to children inappropriately. This poses a potential health, safety or personal rights risk to persons in care.

Personal Rights — Based on interview and observations Licensee and S4 have been observed handling children in a rough manner. This poses a potential health, safety or personal rights risk to persons in care.

Mandated reporter training — Based on record review, D1 mandated reporter training expired on 12/16/25 which poses/posed a potential health, safety or personal rights risk to persons in care.

Personnel Requirements — Based on interview and record review, D1 pediatric cardiopulmonary resuscitation and pediatric first aid certification (CPR/First aid) expired on 12/9/25. S1 has a current CPR/First Aid certification but leaves the facility at 2pm, leaving the facility without at least one certified staff member for two hours which poses/posed a potential health, safety or personal rights risk to persons in care.

License and special permit issuance — This requirement is not met as evidenced by: Based on record review, the licensee did not comply with the section cited above in 1 out of 5 files reviewed did not contained a signed LIC 9224, which poses/posed a potential health, safety or personal rights risk to persons in care.

Notice to parents that site visit reports are available — Based on interviews, both L1 and D1 confirmed guardians of a newly enrolling child did not receive copies any licensing reports that documents any Type A citations. This poses a potenial health and safety risk to the children in care.

Buildings and Grounds — Based on observation, and interview the licensee did not comply with the section cited above in which one of the on limits rooms "Kindergarten room" was not clean and safe for the children in care. The room was being used a storage area to include a large massage chair, 1 large plastic bag and broken cardboard box full of items, a vacuum cleaner, a large rubbermaid container that had chicken bedding inside, as well as a heat lamp attached to the back of a child sized chair pointing to a metal cage which housed two chickens. The heat lamp was accessible to children and was hot to touch, which poses/posed a potential health, safety or personal rights risk to persons in care.

(a) In addition to the sign-in procedure requirement of Section 101226.1(b), the licensee shall develop, maintain, and implement a written procedure to sign the child in/out of the child care center that shall, at a minimum, include the following: (1) The person who signs the child in/out shall use his/her full legal signature and shall record the time of day. This was not met as evidenced by: During LPA's review of the children's sign in/out procedures, it was revealed they were incomplete, missing the full legal signature of the persons signing the child in/out.The licensee did not comply with the section cited above which poses/posed a potential health, safety or personal rights risk to persons in care.

Notice to parents that site visit reports are available — Based on interviews it was revealed that parents of children receiving services where not provided the entire licening report dated 08/02/2024 that documented a Type A citation. This poses a potenial health and safety risk to the children in care.

Responsibility for Providing Care and Supervision — During interview and record review two children C1 and C12 were served items in which they are not to consume.

Personal Rights — Based on interviews multiple children been observed sleeping in areas other than those designated for napping.

Limitations on Capacity and Ambulatory Status — LPA observed one school aged child leaving the facililty at 8:18 am after being in care, the licensee did not comply with the section cited above which poses/posed a potential health, safety or personal rights risk to persons in care.

Limitations on Capacity and Ambulatory Status — LPA observed one school aged child co-mingling with the preschool children while eating lunch, the licensee did not comply with the section cited above which poses/posed a potential health, safety or personal rights risk to persons in care.

Personal Rights — During interviews it was revealed that (S1) was with two children on the floor when (C1) tried to bite S1. S1 stated very loudly “do not bite” and then picked up/pulled C1 in a rough manner and then placed C1 away from them.

Child's Records — Based on LPA's review of seven children's records which revealed C1-C7's records were missing various required licensing forms and were incomplete. This poses a potential health, safety and or personal rights risk to the children in care.

Reporting Requirements — Based on interview facility has not reported any incident’s to CCL involving children. CCL has not received any reports in 2022 or 2023, and there have been incident’s that children have gotten injured and required medical attention. This poses a potential health, safety or personal rights risk to persons in care.

Observation of the Child — Based on adult interviews incidents have occurred at the facility in which their children sustained minor or unexplained injuries and were not informed. This poses a potential health, safety or personal rights risk to persons in care.

Daily Inspection for Illness — Based on interview and observation children are attending the facility while ill. The facility is not checking the children daily for obvious signs of illness. This poses a potential health, safety or personal rights risk to persons in care.

Responsibility for Providing Care and Supervision — Based on interviews and observations the facility is not providing supervision to children while in care, which poses a potential health, safety or personal rights risk to persons in care.

Current roster of children — Based on record review, interviews and observations the roster was incomplete upon request. This poses a potential health, saftey or personal right risk to persons in care.

1596.8662(b)(3) On and after January 1, 2018, a person who becomes an administrator or employee of a licensed child day care facility shall complete the mandated reporter training ... within the first 90 days that he or she is employed at the facility and shall complete renewal mandated reporter training every two years following the date on which he or she completed the initial mandated reporter training. This is not evidence by: Staff A1 and A4 had not taked the Mandated Reporter training upon being hired and within the 90 days. A3 had an expired Mandated Reporter Certificate 07/2023 and did not renew within the 2 years as required.

Drinking water lead testing — Based on observation, interview and record review, the licensee did not comply with the section cited above in which they did not complete the lead testing which poses/posed a potential health, safety or personal rights risk to persons in care.

Child Care Center Director Qualifications and Duties — Based on observation, interview and record review, the licensee did not comply with the section cited above in which they do not have a qualified center director which poses/posed a potential health, safety or personal rights risk to persons in care.

Personnel Requirements — Based on observation, interview and record review, the licensee did not comply with the section cited above in which S3 did not have a health screening, or TB test which poses/posed a potential health, safety or personal rights risk to persons in care.

Teacher Qualifications and Duties — Based on observation, interview and record review, the licensee did not comply with the section cited above in which she is not a qualifed teacher, which poses/posed a potential health, safety or personal rights risk to persons in care.

Child's Records — Based on observation, interview and record review, the licensee did not comply with the section cited above in which C1 - C7 did not have complete files with required documents, which poses/posed a potential health, safety or personal rights risk to persons in care. Documents missing are the Admission Agreement, LIC613A,LIC700,LIC701, LIC995, LIC627, and proof of immunizations.
