Family home — home free of safety hazards — Based on LPA Calderon observation Raid Poison bottle; the raid bottle was immediately placed in a high to reach cabinet located above the stove until deficiency is corrected and poison gets locked up, picture taken, the licensee did not comply with the section cited above in [1] out of [1] poison bottle which poses an immediate health, safety or personal rights risk to persons in care.
Family home — preventive health training, including pediatric CPR and first aid — Based on record review Staff #1 - Facility Representative file was reviewed and was missing Pediatric 1st aid and Cpr, therefore is not a qualified to be left alone with children do to being a liability, and LPA observed this individual left alone with children without Licensee from LPA arrival at 10:20am -11:52am, the licensee did not comply with the section cited above in [1] out of [1] incomplete qualification records for staff to be left alone which poses an immediate health, safety or personal rights risk to persons in care.
(a) Each child receiving services from a family child care home shall have certain rights that shall not be waived or abridged by the licensee regardless of consent or authorization from the child's authorized representative. These rights include, but are not limited to, the following: (2) To receive safe, healthful, and comfortable accommodations, furnishings, and equipment.

Large family home licensing requirements — Based on obsevration at 11:46am Carbon monoxide was tested and indicated fault, picture taken of device,, the licensee did not comply with the section cited above in [1] out of [1] safety devices which poses a potential health, safety or personal rights risk to persons in care.
Family home — home free of safety hazards — Based on observed by LPA Calderon, kitchen was observed, the fire extinguisher is dated 12/7/2020. LPA informed Licensee must get serviced or get a receipt of purchase, yearly to comply with State Fire Marshall Regulations and must be classified 2A10BC or higher., the licensee did not comply with the section cited above in [1] out of [1] fire extinguisher which poses a potential health, safety or personal rights risk to persons in care.
Staff immunization — influenza, pertussis and measles — Based on record review Staff #1 did not have immunization records in file for MMR, TDAP , Flu (can decline) and TB, deficiency cited, the licensee did not comply with the section cited above in [1] out of [3] staff records incomplete which poses a potential health, safety or personal rights risk to persons in care.
Child's Records — Based on observation/record review regarding children records, Child #1 and Child #2 no file for children and no documentation's provided. Child #3 file incomplete missing docs, the licensee did not comply with the section cited above in [3] out of [3] children records, which poses a potential health, safety or personal rights risk to persons in care.
Infant Safe Sleep — Based on LPA Sanchez and LPA Calderon observation and record review Licensee was unable to provide documenation due to the interview with licensee stated not having a log for infants records. The licensee did not comply with the section cited above in [2] out of [2] childrens (Child #3 and Child #4) which poses a potential health, safety or personal rights risk to persons in care.
102416.2(a) Reporting Requirements (a)The licensee shall report the following information the Department by telephone or fax within the Department's next business day and during normal working hours (8am to 5pm). The requirement was not met as evidenced by: based on LPAs observations. Licensee did not report contruction in the home. This poses a potential risk to the health and safety of children in care.
