Infant Safe Sleep — Based on observation and Licensees admission, the licensee did not comply with the section cited above in that there were not enough cribs for the infant present, which poses an immediate health, safety or personal rights risk to persons in care. During todays inspection four infants were present and two cribs/play yards were observed. Additionally, Licensee stated they will use the couch for sleeping infants.

Mandated reporter training — Based on record review, the licensee did not comply with the section cited above in that A1 was missing their mandated reporter certificate, which poses/posed a potential health, safety or personal rights risk to persons in care.
Family home — home free of safety hazards — Based on record review, the licensee did not comply with the section cited above in that the roster on file was not current, which poses/posed a potential health, safety or personal rights risk to persons in care. 4 out of the 10 children present were missing from the roster.
Infant Safe Sleep — Based on record review, the licensee did not comply with the section cited above in that 4 out 4 infants present did not have a current sleep log on file, which poses/posed a potential health, safety or personal rights risk to persons in care.
