Personal Rights — The licensee did not comply with the section cited above in that Staff #1 physically moved Child 1 and Child 2 by grabbing them under their arms and pulling them across the room which poses an immediate health, safety, or personal rights risk to persons in care.

Responsibility for Providing Care and Supervision — Based upon information received from the interview with Staff #1 and written statements from staff and witnesses, it was determined that Staff #1 left children without the supervision of a teacher during the incidences that occurred on 05/01/2025, 09/15/2025, and 09/19/2025 which poses an immediate risk(s) to the health, safety, or personal rights of children in care.

Personal Rights — Based upon information received from the interview with Staff #1 and written statements from staff and witnesses, it was determined that Staff #1 violated the person rights of children in care during the incidences that occurred on 06/20/2025, 09/22/2025, and 10/06/2025 which poses an immediate risk(s) to the health, safety, or personal rights of children in care.

Responsibility for Providing Care and Supervision — interviews conducted and disclosures made indicating that Staff #2 was not providing adequate care and visual supervision when Child #1 was left alone in the classroom when the other children were taken to the outdoor play yard which poses an immediate health, safety, and personal rights risk to children in care.

Personal Rights — interviews conducted and disclosures made indicating that Staff #4 violated the personal rights of Child #2 when Child #3 observed Staff #4 pulled on Child#2 hoodie in order to get Child #2 to lay back down on their cot during nap time and Staff #4 violated the personal rights of Child #2 when Staff #5 and Staff #6 observe Staff #4 disclosed that in reaction to Child#2 stating to Staff #4 “I hate you.” Staff #4 responded “I could hate you too.” which poses an immediate health, safety, and personal rights risk to children in care.

Responsibility for Providing Care and Supervision — interviews conducted and disclosures made indicating that child #2 was not observed biting various children on various dates which poses an immediate health, safety, and personal rights risk to children in care.

Based on record review, Staff 1 did not have an immunization record for pertussis and measles. The licensee did not comply with the section cited above in 1 of 1 Teachers does not have current immunization record on files which poses a potential health, safety or personal rights risk to persons in care.

The incident involves a two year old child wetting on herself and not having a change of clothing and staff had to find clothing around center to change child. Child is to have changes of clothes at center this can pose a health & safety hazard.

Personal Rights — disclosures made when Staff #1 grabbed Child#1 by the arm to make Child #1 nap and observations made Staff #2 and Staff #3 when Staff #1 was observed grabbing Child #2 from under the arms and making Child #2 sit on a chair which poses a potential risk to the health and safety of children in care.

Personal Rights — observations made by LPA when Staff #1 was observed grabbing and holding children by the arms and LPA observing Staff #2 holding Child #1 down on the floor by the arms which poses a potential risk to the health and safety of children in care.

Responsibility for Providing Care and Supervision — interviews conducted and disclosures made indicating that Staff #3 was not providing adequate care and visual supervision when Child #1 was left alone standing near the restroom without supervision. This is an potential risk to the health and safety of children in care.

Personal Rights — interviews conducted and disclosures made indicating that staff #2 withheld child #1 to prevent child from throwing rocks which poses a potential health, safety, and personal rights risk to children in care.
