At 9:00 AM during document reviews, LPA observed that the last fire drill was conducted on 11/30/2024, which posed a potential health, safety or personal rights risk to persons in care.
Buildings and Grounds — Based on observation, interview, record review, The licensee did not comply with the section cited above. At 9:00 am, during the conduct inspection and interview, Director. LPA observed that room 1 and room 2 have Lysol wipes and Sunscreen spray (Alba Botanica) in their classroom children's counter without the children present, which posed a potential health, safety or personal rights risk to persons in care.
Mandated reporter training — Based on observation, interview, and record review. The licensee did not comply with the section cited above in. At 9:00 am, during the file review and interview, Director, LPA observed that 4 out of 5 reviewed staff files did not have current Mandated Reporter training certificates and were expired, which posed a potential health, safety or personal rights risk to persons in care.
Disaster and Mass Casualty Plan — Based on observation, interview, and record review the Director did not comply with the section cited above in The Director confirmed she did not conduct a disaster drill within the past 6 months, which poses/posed a potential health, safety or personal rights risk to persons in care.
Mandated reporter training — Based on observation, interview, record review, The did not comply with the section cited above in 1 out of 5 staff member.
