Based on the record review and observation, the Licensee did not meet the above regulation which poses an immediate safety risk to the children in care. During the facility tour LPA observed S7 providing Supervision & Care to children in care as a Lead Teacher. During file review the LPA was unable to review transcripts with required courses for S7. 101229 Responsibility for Providing Care & Supervision, (a) The licensee shall provide care & supervision as necessary to meet the children's needs. (1) No child(ren) shall be left without the supervision of a teacher at any time... to include visual observation. This regulation was not met as evidenced by: Based on observation, record review, interview and by admission of the licensee, on 08/26/2025 a subject staff left a subject child unsupervised in a classroom for 2-3 minutes, which is an immediate risk to the health and safety of children in care.
