Infant Safe Sleep — Based on observation, interview and record review, the licensee did not comply with the section cited above due to LPA and LPM observed that the Licensee did not have a record of checking for or logging labored breathing, temperature, position, flushed skin color, and restlessness every 15 minutes which poses a potential health, safety or personal rights risk to persons in care.
Alterations to Existing Building or Grounds — Based on observation, interview, record review, the licensee did not comply with the section cited above due to LPM and LPA observing that the facility sketch does not match the actual configuration of the home. It also notes that Room 3 is accessible, however, during the visit Room 3 is not accessible to children in carewhich poses/posed a potential health, safety or personal rights risk to persons in care.
