In the course of the investigation, it was determined that some parent/legal guardians had been informed that the facility closes at 5:30pm and/or earlier than 6:00pm, which differs from the parent handbook and facility license, which state 6:00pm. This poses/posed a potential health, safety or personal rights risk to persons in care.
Personnel Requirements — Based on record review, the licensee did not comply with the section cited above. In reviewing staff records, staff #10 was hired with a TB test/Health Screening that had been conducted over a year ago. Notice to parents that site visit reports are available — Based on record review, the licensee did not comply with the section cited above. In reviewing children files, Child #4, #5, #6, #7, #8, #11, and #12, were missing an Acknowledgement of Receipt of Licensing Reports (LIC9224) for a Type A citation that was issued on 8/14/2025. This poses/posed a potential health, safety or personal rights risk to persons in care.
Written directives for drinking water lead testing — Based on record review the facility has not completed the testing of their water, that is used for consumption, for lead. This poses/posed a potential health and safety or risk to persons in care.
