Mandated reporter training — Based on the records review and interviews, the licensee did not comply with the section cited above because two English teachers (S1 and S2) did not have valid mandated reporter training certificates, which poses a potential health, safety or personal rights risk to persons in care.
Staff immunization — influenza, pertussis and measles — Based on interview and record review, the licensee did not comply with the section cited above because one teacher present (S3) cannot provide her measles record, which poses a potential health, safety or personal rights risk to persons in care.
