During record review, LPA observed licensee last fire/disaster drill was documented on 2/4/2022 which poses/posed a potential health, safety or personal rights risk to persons in care.

While touring the facility, LPA observed a child placed inside a bouncer that was located inside a crib which poses/posed a potential health, safety or personal rights risk to persons in care.

Licensee was unable to provide LPA documentation showing she is documenting and checking on infants every 15 minutes which poses/posed a potential health, safety or personal rights risk to persons in care.

Licensee was unable to provide a copy of her Mandated Reporter Certification which poses/posed a potential health, safety or personal rights risk to persons in care.
Family home — preventive health training, including pediatric CPR and first aid — Based on observation, interview, and record review, the licensee did not comply with the section cited above. Licensee was unable to provide a current Pediatric CPR and First Aid Cerification. Licensee certification expired 4/18/2022 which poses/posed a potential health, safety or personal rights risk to persons in care.

During file reviews, LPA observed child (C1) did not have an Individual Infant Sleeping Plan (LIC 9227) form which poses/posed a potential health, safety or personal rights risk to persons in care.
