Family home — home free of safety hazards — Based on observation, the licensee did not comply with the section cited above. LPA observed storage of hazardous chemicals in an off-limit area an infant was left in. Furthermore, LPA found a generous amount of batteries stored in a drawer the height of the children in care. Under the kitchen sink cabinet, LPA observed two half gallon window cleaners behind a latch, not lock. This poses an immediate health, safety or personal rights risk to persons in care.

Alterations to Existing Building or Grounds — An infant was placed in an off-limit bedroom of the home, which was not approved by the department. During LPAs visit, LPA toured the bedroom and found storage of hazards: cleaning chemicals, shampoos, and soaps. The closet had multiple items stacked on top another, which can be dangerous for the age of the children in the daycare.This poses an immediate health, safety or personal rights risk to persons in care.

Family home — home free of safety hazards — Based on LPAs observation, the licensee did not ensure that hazards were inaccessible to children. LPA found batteries in a drawer at the level of children, a cracked garbage bin, 2 half gallons of window cleaner solution behind a latch, and storage of cleaning chemicals and storage of shampoos, and soaps in an area there was a child present. This poses/posed a potential health, safety or personal rights risk to persons in care.

Family home — home free of safety hazards — Based on record review, the licensee was unable to provide proof of disaster drill logs. This poses/posed a potential health, safety or personal rights risk to persons in care.

Family home — home free of safety hazards — LPA observed a baby walker in the outdoor off-limit area of the home. Although licensee stated she has never used the walker and the walker looked really old and not safe for use, licensee was made aware that there should never be a baby walker allowed on the premises of a family child care home. This poses/posed a potential health, safety or personal rights risk to persons in care.

Infant Safe Sleep — An infant was inside 1 out of 2 of the cribs LPA observed today. LPA did not see a fitted sheet in the crib the 'sick infant' was in. Licensee kept the child separated, however, will not be able to clean/wash the infected area, as there was no sheet. This is being downgraded to a Type B citation, since licensee has another option via another crib for infant sleep, if needed. This poses/posed a potential health, safety or personal rights risk to persons in care.

Infant Safe Sleep — Based on licensees own admission, an infant was placed in an off-limit bedroom of the home with the door closed. When the infant came out, the infant looked flushed and hair looked slightly damp. Licensee stated the infant had a fever, however, the infant was later observed to be okay. LPA cannot prove the infant was sleeping, therefore this is being downgraded to a Type B. This poses/posed a potential health, safety or personal rights risk to persons in care.

Immunizations — Based on record review, the licensee did not comply with the section cited above. 5 out of 5 children's files did not have Immunization Record (IR) on file. This poses/posed a potential health, safety or personal rights risk to persons in care.

Family home — home free of safety hazards — Based on observations, the licensee did not comply with the section cited above as they did not ensure stairs to the second floor were fenced or barricaded which poses a potential safety risk to children in care.
