Buildings and Grounds — Based on LPA observations and Site Supervisor and Head Teacher Leslie Pulido, the licensee did not comply with the section cited above. LPA observed 2 unsecured drop ceiling tiles located in the back corner of the classroom. Furthermore, the knob for the PS bathroom is missing which poses/posed a potential health, safety or personal rights risk to persons in care.
Staff immunization requirements — Based on LPA records review and Site Supervisor and Head Teacher Leslie Pulido interview, the licensee did not comply with the section cited above. S1 and S2 does not have proof of their flu shots in file which poses/posed a potential health, safety or personal rights risk to persons in care.
Child's Medical Assessments — Based on LPA records review and Site Supervisor and Head Teacher Leslie Pulido, the licensee did not comply with the section cited above. C2, C4 and C5 does NOT have LIC701 in file which poses/posed a potential health, safety or personal rights risk to persons in care.
