Based on LPA's observation, the home was cluttered, has stains on the chair cushions, dirth dishes on the counter and in the sink, and dirty stove. Infant Safe Sleep — Based on observation and record review, the licensee did not comply with the section cited above in C1 not having 15 minute sleep log checks on file and statses they do not conduct 15-minute checks on any infants which poses a potential health, safety or personal rights risk to persons in care.
Staff immunization — influenza, pertussis and measles — Based on record review, the licensee did not comply with the section cited above in 2 out of 3 staff do not have immunizations on file which poses a potential health, safety or personal rights risk to persons in care.
Immunizations — Based on record review, the licensee did not comply with the section cited above in 4 out of 5 children do not have immunizations and/or TB assessment test on file which poses a potential health, safety or personal rights risk to persons in care.
Family home — home free of safety hazards — Based on record review, the licensee did not comply with the section cited above in 2 out of 5 children do not have consent for medical treatment on file which poses a potential health, safety or personal rights risk to persons in care.
Infant Safe Sleep — Based on observation, the licensee did not comply with the section cited above, which poses/posed a potential health, safety or personal rights risk to persons in care. Specifically, LPA observed a basinet attachment on a pack n' play which licensee stated she used for napping. LPA observed another basinet in another room. LPA did not observe children napping in the basinets.
