Criminal Record Clearance — During today's visit, LPAs discovered that Staff 4 has been employed and working at this facility since March of 2022, prior to obtaining a criminal record clearance. Based on observation, interview and record review, the licensee did not comply with the section cited above, which poses an immediate health, safety or personal rights risk to persons in care.

Drinking water lead testing — Joyce's Toluca Lake Pre-school did not complete Lead testing prior to their deadline. Based on observation, interview and record review, the licensee did not comply with the section cited above, which poses a potential health, safety or personal rights risk to persons in care.
Mandated reporter training — The Director's mandated reporter training had expired on 3/27/2020. Staff 2 and 3 were missing the mandated reporter training certificates. Based on observation, interview and record review, the licensee did not comply with the section cited above, which poses/posed a potential health, safety or personal rights risk to persons in care.
Immunizations — During file review, LPAs observed immunization records were missing for Child 2 and Child 3. Based on observation, interview and record review the licensee did not comply with the section cited above, which poses/posed a potential health, safety or personal rights risk to persons in care.
Child's Records — During file review, LPAs observed the following forms were missing for Children 1 - 3. Child 1 was missing LIC 701. Children 2 and 3 were missing LIC 613A, LIC 700, LIC 701, LIC 995 and LIC 627. Based on observation, interview and record review, the licensee did not comply with the section cited above, which poses/posed a potential health, safety or personal rights risk to persons in care.
