Food Service — Based on observation and interview, the licensee did not comply with the section cited above in LPA observed kitchen trash cans (2) to not be clean from the outside and missing trash can lid covers which posed a potential health, safety or personal rights risk to persons in care.
Alterations to Existing Buildings or New Facilities — Based on LPAs observation LPAs viewed unauthorized new construction of a kitchen area and removal of two restrooms on the day of inspection which posed a potential health, safety, or personal rights risk to the children in care.
Responsibility for Providing Care and Supervision — Based on LPAs interviews and record review, infant classroom staff did not notice a child having a piece of plastic in their mouth which posed a potential healthy and safety risk to the children in care.
